Rhinophyma

Rhinophyma is a skin disorder that causes the nose to enlarge and become red, bumpy, and bulbous. It is thought to result from severe roseacea that causes facial redness on the nose and cheeks.

Causes and Risk Factors of Rhinophyma

The exact causes of rhinophyma are unknown, but certain factors can increase the risk. These risk factors include:

  • Age: Rhinophyma is typically diagnosed between age 55 and 75.
  • Gender: Men are more likely to develop the condition than women.
  • Complexion: Fair-skinned people are more likely to have rhinophyma.
  • Ethnicity: People with an Irish, English, Scottish or Scandinavian, background are at a higher risk.
  • Family history: People with family members who have had rosacea or rhinophyma are also at a higher risk.

Symptoms of Rhinophyma

Rhinophyma often develops in severe rosacea. Its presentation can occur in cycles and it become worse if left untreated. Features include:

  • Gradual changes in appearance of the nose to a swollen, bulbous shape
  • Many visible oil glands and enlarged pores
  • Reddish skin color
  • Thickened skin
  • Waxy, cobblestone, yellowish surface

Diagnosing Rhinophyma and Initial Consultation

Our experienced plastic surgeon Dr Richard Hamilton conducts a thorough evaluation, which includes a:

  • Physical exam of the nose from all angles
  • An assessment of the deeper structure of the nose (bone and cartilage)
  • Airway assessment
  • Photographs are taken and diagrams are made
    Also Vectra 3d imaging is taken and this will allow Dr Hamilton to simulate the results on screen to give you an idea of what might be achieved.
  • Discussion of personal and family medical history


Usually, Dr Hamilton can diagnose rhinophyma with this information, without the need for further testing. 2 Consultations are preferred.

About Treatment for Rhinophyma

For rhinophyma, surgery is the main goals of treatment are to:

  • Reshape a disfigured nose
  • Remove tissue overgrowth
  • Improve appearance and breathing ability
  • Dr Hamilton will discuss with you the procedure in great depth and allow ample opportunities for any questions you may have.
  • Dr Hamilton’s approach treatment consists of
    shaving down the bulk of the tissue with special surgical instruments
    to sculpture and reshape the nose and restore its natural shape.


Rhinophyma surgery can be performed under local anaesthesia or twilight sedation. Then Dr Hamilton will allow the area to heal by natural regeneration assisted with monitored dressings. In very rare cases, skin grafting may be needed.

It was once believed that rhinophyma was caused by heavy alcohol use, but that is not correct. Rhinophyma occurs just as frequently in people who drink no alcohol and those who drink heavily. The condition can lead to permanent disfigurement, and a large bulbous nasal tip can cause nasal obstruction. It is eminently treatable by surgery.
